Choosing forms and sizes
Which pieces do I need?
Straight forms handle straight runs. Flexible forms handle curves. Corners make a 90-degree turn. Five-foot fillers slide over straight forms to help close odd-length gaps. If you are not sure, call before you order.
How do I read the form size?
The first number is the form height and the second number is the piece length. A 6-inch by 10-foot form is 6 inches high and 10 feet long. Corner sizes show the height of the Ohio Metal Forms straight section they fit.
Does quantity mean pieces or feet?
Quantity means individual pieces, not feet. One straight or flexible form is one 10-foot section. One filler is one 5-foot section. One corner is one 90-degree corner.
What form heights are available?
The current standard straight, flexible, and filler sizes are 4, 6, 8, 10, and 12 inches high. Matching corner heights are listed on the corner product page. Check the live size selector for current prices.
How do I choose the form height?
Choose the height required for the concrete edge on that part of the job. This website cannot determine the structural design of your slab or project. Follow the plans and applicable requirements, and call us if you need help matching a listed form size.
Can I mix heights in one order?
Yes. Add each height and quantity separately. Corners and fillers should match the height of the Ohio Metal Forms straight sections they connect to.
What is the minimum radius for a flexible form?
The practical radius depends on form height and field setup. Call with the height and planned radius before ordering for a tight curve.
Will these parts fit forms I already own?
Do not assume parts from different forming systems will match. Ohio Metal Forms corners are made for the female end of Ohio Metal Forms straight sections. Call with photos and measurements before mixing systems.
How much does each form weigh?
Weight depends on the product and size. Call with the exact product and height if you need weight for handling, transport, or freight planning.
Planning quantities and hardware
How many 10-foot sections should I order?
A typical run needs forms on both sides. Divide the run length by 10, round up, and multiply by two. For example, a 100-foot run starts with 20 ten-foot forms: 10 per side. Separate straight runs from curved runs, calculate each form height separately, then account for corners, connections, odd-length adjustments, and jobsite conditions. For pads, closed shapes, or mixed layouts, call before paying so we can check the list.
Can I order a length that is not a full 10 feet?
The five-foot filler slides over a straight form to help adjust a run without cutting the main form. Call about any other non-standard requirement.
What is included with one quantity?
Each quantity adds one listed form piece in the selected size. Pins, stakes, and other jobsite hardware are not automatically added.
Are pins or stakes included?
No. Only the products and quantities shown in your order are included. Call before paying if you need help identifying additional jobsite hardware.
Can someone look over my order?
Yes. Call 419-515-2290 before paying. Have the heights, straight and curved footage, quantities, corners, fillers, and pickup or delivery plan ready.
Do you handle large or repeat orders?

Ordering, production, and payment
What is the fastest way to order?
Use Quick Order. Every standard form, height, and price is on one screen. Enter quantities, choose pickup or freight, and continue without creating an account.
Are the forms in stock?
Usually not. Most forms are made to order. Typical production time is about 7–8 business days, and we confirm the exact ready date after reviewing the order.
When does production start?
We review the submitted order and payment status, then confirm the schedule. Contact us promptly if a date is critical; a website estimate is not a guaranteed ready date.
How do custom quotes work?
Ahner Industrial emails a private link showing the forms, freight, applicable tax, and complete total. Review it and pay securely on the Ohio Metal Forms website before the link expires.
Do quantity price breaks apply automatically?
Yes. The current straight-form price breaks are 7% off at 50 pieces, 8% off at 100 pieces, and 9% off at 150 pieces. Straight forms of different heights combine toward the qualifying quantity. Once a tier is reached, its discount applies to all products in that order. The site shows the savings before checkout; freight is priced separately.
What payment methods can I use?
Available methods appear in the secure Stripe payment form. Card payment is supported, and eligible devices may also show wallet options such as Apple Pay, Google Pay, Link, or Amazon Pay.
Will I receive an order confirmation?
Yes. The site emails order and payment updates to the address you enter. Check the address carefully before submitting and look in spam or junk if an expected message is missing.
Is sales tax charged?
Applicable tax is shown before payment. Local pickup currently uses the 6.75% combined Sandusky, Erie County, Ohio rate. Freight-order tax is reviewed with the delivery details.
Can I change an order after payment?
Changes after payment are not guaranteed because most forms are made to order and all sales are final. Contact us immediately; unpaid orders and quotes can be cancelled.
Pickup and freight delivery
Can I pick up my order?
Yes. Pickup is Monday through Friday, 8:00 a.m. to 4:30 p.m. Wait for the ready-for-pickup email and pickup instructions before arriving at 2001 E Perkins Ave, Sandusky, Ohio.
What should I bring for pickup?
Bring the order information and a vehicle or trailer suitable for the quantity and length of the forms. Secure the load correctly. Call ahead if you need exact product weights or help planning the vehicle.
Do you deliver by freight?
Yes. Enter quantities, choose freight delivery, and provide the complete delivery address. Checkout compares eligible USPS, XPO, and Ahner truck pricing, then shows the lowest available delivered total before secure payment. Ahner truck delivery is considered only when customer unloading equipment is available; it cannot provide liftgate service.
What information is needed for freight pricing?
Provide the complete address, delivery ZIP code, business and contact name, phone number, whether the site is commercial or residential, and whether a dock, forklift, or liftgate is needed. Mention limited-access or jobsite conditions.
How long does freight take?
Carrier transit time is separate from production time and depends on destination and service. The quote or order update will provide the information available for that shipment.
What if freight arrives damaged or short?
Inspect the shipment before signing when possible. Note visible damage or shortages on the carrier paperwork, keep photos and packaging, and contact us promptly.
Use, care, and policies
What are the forms made from?
Straight, flexible, and filler form bodies use 12-gauge ASTM A1011 CS Type B USA domestic carbon steel. Wedge-pocket components use 3/16-inch and 1/4-inch steel of similar composition. Form corners use 12-gauge USA domestic steel.
How should I care for steel concrete forms?
Clean concrete residue promptly, use a suitable release product according to its instructions, inspect the form and connections before reuse, and store forms securely in a dry location. Remove damaged equipment from service until evaluated.
Are returns accepted?
No. Due to the nature of these products, all sales are final. Verify size, quantity, compatibility, and fulfillment details before paying.
Where is the safety information?
The safety data sheet is available from the Resources page and each product page. Crews are responsible for appropriate training, handling, personal protective equipment, and jobsite practices.
